It takes about 20 minutes for a new patient to fill the required forms.  This can be done at our clinic, or you can save your wait time by completing all the forms before you arrive to our clinic.

¨ Please fill all required forms below (marked with *).

¨ Please choose correct Colorado Mandatory Disclosure Statement for Practitioner you are seeing.

¨ If you have Microsoft Word, please open the file, update the form while your session is open.  Warning: if you close your session before printing, you will lose your data.

¨ If you don’t have Microsoft Word, you can use the PDF format.  Print the form and fill it out using a pen.  If you don’t have Adobe Acrobat Reader, you can download it for free from here.
